1. First go to this link -I am Eligible
   
2. After enter your mobile number you receive one otp enter that OTP an
   click on submit button
3. Choose your state and category
   • You can search by Name
   • You can search by mobile number
   • You can search by Ration card number
4. Enter your mobile/name/ration number
5. If list already contains your name it will appears on right side on screen .
6. Then click on one by one family member name and link their Aadhaar card number
7. After enter all family details click on submit button and your all family
   covered under this scheme
8. Print your "Ayushman Bharat Card".
